Our Service Navigator provides care coordination for clients who have an array of need. Many
of these needs fall into a category considered “gaps in service”, meaning there is no specific
program or agency that can meet the need. The Navigator utilizes creativity and forges
connections with other service providers to offer solutions to those in difficult financial
situations. The Navigator can also help guide people through complicated and sometimes
overwhelming enrollment processes for government-funded assistance such as SNAP, Medicaid,
Social Security, Temporary Assistance, and cell phone services.
